In order for our coaches/volunteers to effectively meet the needs for their entire team, does your child have any KNOWN ALLERGIES or PRE-EXISTING health issues that may affect his/her ability to fully participate? This would include any food allergies, drug allergies or prior sports injuries requiring medical treatment.
Fees, Liability, Consent, Parent Support
I, the undersigned parent(s) or guardian(s) of the named participant, understand that this team will require my support and involvement. I am expected to help support the above named participant’s team by volunteering for activities required to make football and cheerleading a success for all children participating. Examples include but not limited to, chain gangs, clock operator, announcer, monitor, passing the bucket, concession, etc.
I, the undersigned parent(s) or guardian(s) of the above named participant, understand that the registration fee that is required for each participant is used to pay the cost of insurance, uniforms and equipment necessary to provide a safe environment for the participant and other miscellaneous costs and activities. All equipment is the property of the individual organization and must be returned. If not, the participant will be prohibited from any future participation with any association within West Texas Youth Football Assoc.; all legal steps to recover equipment may be taken. Registration fees in no way assure a participant in every play of each game. The number of plays depends on the participant’s abilities, attitude, quality and quantity of practice. Every participant will play at least six plays of each game, provided they follow the rules and by-laws of their association and the WTYFA.
Liability and Medical Consent
I, the undersigned parent(s) or guardian(s) of the above named participants, hereby give my consent to his/her participation in any and all WTYFA activities during the current season. I do hereby waive, release, absolve, indemnify and agree to hold harmless the respective conference and any other organization that this football program is affiliated; including but not limited to, the organizers, coaches, sponsors, supervisors, participants and persons transporting above named participants to and from such activities, for any claims resulting from injury to my child and understand that each team member is covered by secondary insurance in the event of physical injury to a participant. I hereby grant authority to any physicians to render such medical treatment as said physician deems necessary under the circumstances at the time of treatment. I understand there is risk of serious injury in the youth sports of football and cheerleading and am willingly registering my child for participation in WTYFA knowing these risks.
Permission to Use Photograph
I grant to WTYFA or its representatives the right to take photographs of my child in connection with WTYFA practices, games and events. I authorize, WTYFA, its assignees and transferees to copyright, use and publish the same in print and/or electronically. I agree that WTYFA may use such photographs of my child with or without their name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, social media and Web content.
I, the undersigned parent(s) or guardian(s) to the above mentioned player or cheerleader, agree to participate under the direction and authority of any and all WTYFA league authorities including direction by the WTYFA Executive Committee or Board. I pledge to conduct myself at all times in an adult and sportsmanlike manner during any league activities to include all events, practices and/or games and do nothing that may be viewed by others as offensive or otherwise considered "acts detrimental to the League" as per the discretion of league authorities. Should I be disciplined by league authorities, I agree to respect and obey their authority and judgement. Acts detrimental may include but are not limited to: refraining from all use of foul, abusive, racist or derogatory language directed at anyone or within earshot of the children at all times. I understand any ongoing escalation or threats will immediately precipitate a 911 call to local police or law enforcement to handle the individual(s) or situation deemed detrimental to the league by its authorities. I also understand any verbal assaults, taunting, brawling, instigating fighting, other forms of aggression, threats of physical assault or other similar threats verbally or on social media will be considered detrimental to the league and may result in my immediate ejection from organizations, team responsibilities, league facilities, or other disciplinary action including possible banning from future attendance at any and all remaining league activities for the remainder of the season.
It is also evidence that I understand the above named participant may NOT participate in a game until the West Texas Youth Football Association has received the above named participants birth certificate, report card, league forms and required physical from a physician or certified nurse, etc.
