the undersigned do hereby certify that I am an Independent Contractor in conjunction with my work as a Referee in conjunction with soccer matches organized and sponsored by Soccer Enterprises, Inc. DBA Soccer City, LLC (Soccer City) In conjunction with my work, I state as follows:

1. I am not an employee of Soccer City. I am not entitled to any wages or other benefits which are provided to employees of Soccer City

2. As an Independent Contractor, I utilize my own talents, experience and knowledge in conjunction with the discharge of my referee responsibilities with each assignment offered to me by Soccer City

3. I am free to accept or reject any soccer match assignment wherein I am asked to act as referee

4. I am free to offer my referee services to other soccer leagues not affiliated with Soccer City

5. I am certifying that I am not an employee but as an Independent Contractor and therefore will not be covered under provisions of the unemployment compensation insurance of Soccer City or the workers’ compensation insurance of Soccer City and that any injury or property damage on the job will be my sole responsibility and not Soccer City

6. I fully understand by signing this I am stating that I am acting as my own business as an Independent Contractor and solely responsible for any and all federal, state and local income and withholding tax obligations or contributions imposed by social security, unemployment insurance and workers’ compensation insurance on behalf of myself or associated with the stipends I receive in conjunction with each soccer match for which I provide professional referee services.
