    This means that they both need to agree on the name chanage of the child. This also means that the names of both parents need to be on the deed poll. 

    However, Please note:

    • If either parent is deceased then sole parental responsibility will pass to the sole living parent.
    • If there is a court order giving someone else parental responsibility then their consent must also be obtained.
    • If both parents have parental responsibility, but permission cannot be obtained from one of them, i.e. because they refuse to consent, or because their whereabouts are unknown, then a specific court order need to be obtained.
