    IMPORTANT NOTES BEFORE YOU START!

    Stand Tu Maia referral forms can require information about a child from caregivers, guardians, medical professionals and schools. It is likely you will need to begin a referral and pause it to seek additional information.

    To facilitate the ability to save and come back to a referral, we recommend you create a FREE acccount at https://www.jotform.com/ (the online platform we use for these forms) before you begin the referral form. This extra step is to ensure the referral form, and the client data within it, remains secure.

    Sometimes your IT environment will prevent these forms from being completed, in which case please request a paper referral form from:

    referrals@standtumaia.nz

    This service can only accept referrals from Referral agents such as Medical, Educational or Other Social support orgnisations.

    You cannot refer yourself or a family member to this service.
