  • In the Project Report text box below, please provide the following information:

    Have there been any changes in your project from the original proposal? If so, what were the changes and why? (For example, if the school received private funding and not all of the Adopt-a-School funds were used. Or if there were more or fewer students in the program than anticipated.)

    If this is an Interim Report, when do you expect the grant funds to be fully spent? (If the completion date may have changed from the original application based on any changes noted, this should be explained.)

    How many children have benefited from this program so far? Please describe the impact the funding has made for students, families and the school community (as appropriate).

    If this is a Final Report, a brief summary that we can share with donors, indicating how the program helped students, would be appreciated.

For example, if a staff member has been reimbursed for an expense, the staff person’s name should not be included; however a description of what the expense was for must be shown, for example:
