Consent for Treatment at office location:
I consent to meet with and be interviewed by a Tidewater CFBH provider for the purpose of understanding my service needs.
I consent to participate in a psychological evaluation, if applicable to better understand my presenting problem and for my treatment provider to make informed treatment recommendations about my presenting problem.
I consent to participate in psychotherapy with a Tidewater CFBH therapist. There are many different styles of therapy, but Tidewater CFBH is committed to using evidence based psychotherapies for children, families, and adults that have been demonstrated to be effective in helping. As a result, we typically use cognitive behavioral, behavioral, and family systems approaches to treatment.
Developing and implementing a treatment plan:
Typically, during the first 2 appointments, your therapist will complete an initial intake with you. During this intake, we will obtain information about your family history, your educational and work history, medical history, developmental history, and history of the presenting problem that you are seeking services for. Your therapist will then discuss their findings and recommendations for helping you. At that time, we will collaborate to develop a treatment plan for you.
