Consent for Treatment at office location:
I consent to meet with and be interviewed by a Tidewater CFBH provider for the purpose of understanding my child’s service needs.
I consent for my child to meet with and be interviewed by a Tidewater CFBH provider for the purpose of understanding my child’s service needs.
If recommended by my treatment provider, I consent for my child to participate in a psychological evaluation. The purpose of such an evaluation is to better understand their presenting problem and for my treatment provider to make informed treatment recommendations about my child’s presenting problem.
I consent for my child to participate in psychotherapy with a Tidewater CFBH therapist. There are many different styles of therapy, but Tidewater CFBH is committed to using evidence-based psychotherapies for children and families that have been demonstrated to be effective in helping. As a result, we typically use cognitive behavioral, behavioral, and family systems approaches to treatment.
Developing and implementing a treatment plan:
Typically, during the first 2 appointments, your child’s therapist will complete an initial intake with you and your child both present. During this intake, we will obtain information about your family history, your child’s academic history, medical history, developmental history, and history of the presenting problem that you are seeking services for. Your therapist will then discuss their findings and recommendations for helping your child with you. At that time, we will collaborate to develop a treatment plan for your child.
Depending on your child’s presenting problem, we may recommend that your child participate in individual therapy, that you participate in therapy with your child, recommend family therapy, or some combination of these approaches. After we agree to a treatment approach, we will begin regular therapy sessions.
