Thank you for choosing Chesdin Animal Hospital. Our primary mission is to deliver the best and most compassionate veterinary care available to your pet. While your pet is here, they will receive the necessary diagnostic tests and treatments to ensure proper medical care. Chesdin Animal Hospital requires payment in full at the end of your pet’s visit and /or at the time of discharge. We want you to understand your responsibility for the payment of your account.

FULL PAYMENT IS DUE AT THE TIME OF SERVICE

• We accept cash, check, debit, Visa, MasterCard, Discover, American Express, ScratchPay and Care Credit.
• If you have pet insurance, we are happy to provide you with the necessary documentation to submit a claim to your insurance company. However, we are not responsible for your agreement with the insurance provider.
For sick, injured, emergencies and surgeries, a deposit of 50 % may be required prior to beginning your pet’s treatment.

*Failure to show for appointments or (Grooming, Surgeries, Boarding and if Office appointment are given) without giving 24 hours of notice, will be accessed at $20.00 fee.

Client Responsibility and Addition Terms
Although billing is not prohibited, any accounts with any unpaid balance are subject to a 1 ½% monthly (10% annual) finance charge plus $2.00 statement and handling fee. If we have to submit your unpaid account through a collection process you will be responsible for all charges our practice incurs; including late fees, finance fees (10% or $50.00 whichever is greater), collection cost, court filing fees and reasonable attorney’s fees. Any returned check or credit card payments will carry a $35.00 service charge.

Medications
If your pet is prescribed medication, keep medication out of reach of your pets and children. Never use one pet’s medication for another pet unless you are instructed to do so by our veterinarian. Never use your dog’s medication in your cat, because drugs that may be safe for dogs may not be safe in cats. CAUTION: Medications prescribed are for animal use only, not for human consumption.

*Prescription request that are written, faxed or called in, will be accessed at $10.00 clerical fee prior to any approval (exception is for Chesdinanimalhospital.com Pharmacy).

This contract extends to all pets and treatments today and going forward.
