Please submit proof of COVID-19 vaccination here. All members and guests attending Heartland Fall Forum are required to be fully vaccinated and boosted with a CDC approved COVID-19 vaccine. We will determine our specific mask policy closer to the event so we can work with the most up-to-date information.
All attendees must also read and agree to the COVID-19 Safety Acknowledgment, Liability Waiver and Release of Claims.
A variety of documents will be accepted. All forms of proof must include: 1) Your full name, 2) Name of official source issuing the record (e.g., public health agency, government agency, or other authorized vaccine provider), 3) Vaccine manufacturer and date(s) of vaccination.
