Church Officers' Form
This form will be used to generate a database for the National Auxiliary Presidents, the General Secretary's office, and the National Treasurer's office.

Membership Roll Form
This form will be used by the National Church to determine the total number of members in the organization. The form is set up to enter up to 100 members. If you will need to enter more than 100 members, send an email to brendon.hamner@houseofgod. org before you begin completing the form so a different link can be sent to you.

Pastor's Bio Form
This form will be used by the General Secretary for informational purposes.

Secretary's Bio Form
This form will be used by the General Secretary for informational purposes.

Email Distribution List
This form should be completed if there are individuals at your local church that would like to be added to the General Secretary's national church email distribution list. The form is set up to enter up to 25 email addresses. If you will need to enter more than 25 email addresses, send an email to brendon.hamner@houseofgod. org before you begin completing the form so a different link can be sent to you.
