👉 Form हिन्दी-English दोनों भाषा में उपलब्ध है / आप अपने सुविधा अनुसार भाषा बदल सकते हैं

👉 Star mark(*)  वाले Box को भरना अनिवार्य है, 

👉 बिना star(*) वाले Box  को आप छोड़ भी सकते हैं,अगर आप के पास जानकारी उपलब्ध नहीं है तो

👉 Association को कुछ राशि का सहयोग करने की ईच्छा हो आप में(ex:- 5,10,20,50,100,...) तो  yrsa के  Pament website पर जा कर सहयोग कर सकते हैं
