In General:
- Refunds will not be given for cancellations, suspensions, or unenrollment.
- Credits will not be granted for missed classes.
- In situations of a "Stay-At-Home" or "Shelter-In-Place", including but not limited to possible flooding, assignments might be made available to prevent students from falling behind.
Statement of Faith

Christian Ministries Academy is interdenominational; however, we doctrinally hold to these basic fundamental teachings of God’s Word:
- The inspiration of the Bible, equally in all parts and without error in its origin.
- There is one God, a three part being, who is eternally existent. (Father, Son, and Holy Spirit)
- The pre existence, incarnation, virgin birth, sinless life, miracles, substitutionary death, bodily resurrection, ascension to heaven, and the second coming of the Lord Jesus Christ.
- The fall of man, the need of regeneration by the operation of the Holy Spirit on the basis of grace alone, and the resurrection of all; to life or damnation.
- The spiritual relationship of all believers in the Lord Jesus Christ, living a life of righteous works, separated from the world, witnessing of His saving grace through the ministry of the Holy Spirit.

Parent/Student Commitment
- I understand the importance of regular church attendace and agree to consistently attend.
- We agree absences will be kept to a minimum.
- We understand that it is important to train our children to come to class prepared with all necessary materials and assignments.
- We will make every effort to arrive on time for each on campus day, since late arrivals may disrupt other students.
- We understand Christian Ministries Academy is a ministry resource offered by Christian Ministries Church and agree to pay program dues in a timely manner.
- We will be supportive of the on campus teachers, CMA administrators, and CMC support leaders to provide students with a positive educational experience.
- We will refrain from gossip about the school, its employees or policies, since such behavior is destructive to the school and is not reflective of Christ-like character.
- I agree to be a co-teacher for my child and to fulfill all related responsibilities to the best of my abilities. I take this commitment seriously and understand
  that it will require time and energy.
- I agree to review all home assignments after my child has completed them.
- I agree to discuss differences in philosophy or expectations with the classroom teacher or leadership team in a positive and professional manner.
- Student agrees to listen to teacher and respond in a respectful manner.
- Student agrees to treat others with respect and will ask teacher for help if there is a conflict with another student.
- Student agrees to remain attentive in class and give their full effort in every activity and assignment.

Student Media Consent and Release Form

Throughtout the school year, students will be highlighted in efforts to promote Chrsitian Ministries Church. Students may be featured in promotional videos, brochures, social media platforms, and other types of media.

I, the undersigner, grant Christian Ministries Church and its representatives, administrative team, and ministry leadership team the permission to photograph, print, record my child for use in audio, video, film, or any other electronic digital and printed media. I release Christian Ministries Church, its ministry teachers, administrative team, and Board of Directors from any liabilities, known and unknown, arising out of the use of this material.

I consent for my child/ren to participate in Christian Ministries Academy program. I fully understand Christian Ministries Academy is a ministry program offered by Christian Ministries Church with the sole purpose to instill a biblical worldview and academic growth in each student.

I declare that the information I have provided in this enrollment form is true and correct and confirms my child/ren(s) participation in the Christian Ministries Academy program.

ASSUMPTION OF RISK: I agree that I and my child/ren are voluntarily participating in the activities offered by Christian Ministries Church, but not limited to, the use of the equipment, playground, facilities and the premises. I am assuming on behalf of myself and child/ren, all risk of personal injury or disability to myself and/or child/ren that may result from participation, or any damage, loss or theft of any personal property which me and/or child/ren may incur.

RELEASE OF LIABILITY:

I understand that while all care will be taken, I will not hold Christian Ministries Church, nor any representative, administration, teachers, or leadership team liable for any accident, loss or injury suffered by my child or guardian during the course of their activities with Christian Ministries Church.

I authorize, where it is impractical to contact me, my child receiving, at my expense, any medical or surgical treatment or ambulance costs as may be deemed necessary. I have read and understood the terms and conditions on this enrollment form.

This Waiver and Release of Liability includes, without limitation, injuries, or accidents, which may occur as a result of the: (a) use or misuse of the facility in any way by anyone, (b) use of any equipment that malfunctions or breaks, (c) improper maintenance of the facility, grounds, or any equipment, (d) instruction or supervision, or (e) slipping, tripping and /or falling while in the facility or on the surrounding premises.

I ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS WAIVER AND RELEASE AND FULLY UNDERSTAND THAT IT IS A RELEASE OF ALL LIABILITY AND A WAIVER OF ANY RIGHT THAT I MAY HAVE ON BE HALF OF MYSELF AND MY CHILD/REN TO BRING A LEGAL ACTION OR ASSERT A CLAIM FOR INJURY OR LOSS OF ANY KIND AGAINST CHRISTIAN MINISTRIES CHURCH OF HOUSTON OR ANY REPRESENTATIVE OF CHRISTIAN MINISTRIES CHURCH. SHOULD ANY CLAIM BE MADE, I UNDERSTAND AND AGREE THAT I WILL BE RESPONSIBLE FOR ALL ATTORNEYS FEES AND DEFENSE COSTS INCURRED BY CHRISTIAN MINISTRIES CHURCH OF HOUSTON IN CONNECTION WITH OR IN THE DEFENSE OF THAT CLAIM.
