I am over the age of twenty-one (21) years and am interested in helping Must Luv Dogs Rescue "MLD" as a volunteer foster parent. All of my animals are up-to-date on their vaccines and will be kept safely away from the foster dog until I determine whether or not the foster dog is compatible with my existing family dogs, as well as away from any dogs visiting my family home..  I understand that MLD animals have either been recently vet checked, or are in the process of being checked and vaccinated, for all age appropriate vaccines. I also understand that the history of most foster dogs is unknown and MLD will not be responsible for any inadvertent exposure of foster parents or their animals to unknown disease or other medical conditions. I knowingly accept the risk on behalf of myself and my own animals.    

The undersigned foster parent agrees to the following:

FOSTER herein acknowledges that at NO TIME shall any foster dogs be allowed to go to a dog park, whether private or public.   DOG PARKS are specifically prohibited for use by any and all foster dogs.   Failure to follow this rule may result in expulsion of the foster dog from the MLD program, at the discretion of the director and the board of directors. 

• I agree to contact the foster coordinator or designated veterinarian prior to the 1st of each month to obtain the dog' heartworm preventative, and pick it up and administer it to the foster dog timely, each month.

• I agree to help MLD in the marketing of my foster dog by timely completing the Foster Dog Questionnaire as well as updated photographs and videos on a regular basis. I also agree to share my foster dog on my personal social media channels as well. 

• I agree NOT TO DISCUSS any confidential matters related to my foster dog's history, behavior or medical conditions with anyone outside of MLD's Director, Foster Coordinator or other officers designated by MLD.

• I agree NOT TO SCHEDULE overnight trips or meet & greets without the express consent of MLD's Director, Foster Coordinator or other officers designated by MLD.

• I acknolwedge receipt of and agree to familiarize myself with MLD's Foster Handbook and all of its policies and regulations and to further align myself with the same.

• I further agree to provide MLD with any and all information they may need on the dog at any time, including photographs for the purposes of publicizing the dog’s availability for adoption.

• I agree to be available within a reasonable notice for “meet & greets” with potential adopters. MLD agrees to work with the foster to make these as convenient to all parties as possible.

• Foster agrees to never allow the dog to meet any potential adopter that hasn’t been fully vetted by MLD first and only to meet at a location approved by MLD. MLD agrees to make every effort to make these meet  & greets as convenient as possible to foster.

• Foster agrees that the dog shall never be left unattended outside, or at the home of anyone other than the Foster’s home, without express permission from MLD.

• Foster further agrees that if the dog should go missing or suffer from an illness or accident that Foster shall notify MLD immediately, without hesitation.

• I further agree that if the dog has tested Heartworm positive and is undergoing treatment, either fast-kill or the slow-kill method, that I will provide the dog with a quiet, restful area in which to play, and to follow all instructions, as provided by MLD. If the dog shows any distress, including bloody cough, breathing difficulty, I will contact MLD or the dedicated veterinary clinic as soon as possible.

• I expressly agree to the following tenants:

• I understand that foster dogs must be kept on a leash AT ALL TIMES when in an unfenced or unenclosed area.

• I will never leave my foster dog tied or chained up OR on a "run".

• I will never leave foster dogs in an unattended vehicle.

• I agree to arrange my family’s schedule so that foster animals are rarely left alone for more than eight continuous hours.

• I agree to attend to my foster animal’s needs for socialization and play time and leash training.

• I will not mistreat, abuse, surrender, give away, sell, or euthanize foster animals in my care.

• I will not allow any foster dogs in my care to be trained or used as guard or attack dogs.

• I will notify MLD immediately if any foster animals in my care are lost or stolen and to make immediate, reasonable efforts to locate them.

• I will take adequate care of any foster animal in my possession, providing a:  Safe environment; food; clean drinking water; shelter; bathing and grooming (especially the day of or before adoption events); heartworm/parasite protection and control; administration of medications, treatments, and special diets as determined by the veterinarian.

• I will notify MLD of needed food and medications.

• I will feed and use the food and medications provided by MLD for my foster dog.
  

• I will use the services of the MLD affiliated veterinarian, which information shall be provided to me by MLD.

• I will immediately notify MLD immediately of changes in the health of foster dogs.I will receive authorization from MLD prior to incurring medical expenses for the foster doI will be responsible for the transportation of the foster dog to and from the veterinary office.

• I will maintain the foster dog at the residence listed on this agreement and will notify MLD if I plan to go out of town or relocate.

• I attest that all residents of my household approve my decision to foster animals and will abide by this Foster Agreement.  MLD reserves the right to check on the welfare of the fostered dog on my premises during normal hours, after a reasonable notice from MLD..

• I agree to make my home and my foster dog available during normal hours for viewing by MLD, after reasonable notice by MLD.

• I understand that if I am found to be non-compliant with this agreement, MLD reserves the right to reclaim both possession and ownership of the dog and any food, medications and equipment owned by MLD, without payment of any kind and I waive all claims of trespass related thereto.

• I understand that MLD must approve all adoption applications and I may not promise an adoption of any fostered dog

• I understand that I must make my foster dogs available to prospective owners during normal hours, upon reasonable notice from MLD.

• I herein acknowledge and understand that no matter if I found the dog or if I previously owned the dog or if the dog was provided to me by MLD, I acknowledge that I no longer have any ownership interest in the dog and I herein transfer any & all ownership I may have in the dog to MLD.   I further acknowledge that if I wish to adopt my foster dog aka: "foster fail" that I will notify the Director of MLD as soon as possible; however I acknowledge that the adoption approval is completely up to MLD and that only MLD can approve said adoption, based on what  they feel is in the dogs best interest, in it's sole discretion.  I fully acknowledge that I have NO ownership interest in the said dog and must relinquish possession of the dog to MLD immediately upon request. i acknoweldge that any foster dog adopted by a foster parent is considered a "foster fail" and adoption fees are discounted accordingly, not to exceed up to 50% discount from standard fees, depending on how long the foster family has worked with MLD.

I understand that Must Luv Dogs cannot guarantee the temperament of any animal. There is an adjustment period for any foster animal coming into my home. Foster animals, being living animals, may act unpredictably. As a foster home, I agree to take necessary precautions to prevent accidents and guard against unpredictable actions by the animals in my care. I agree NOT to hold Must Luv Dogs, its members, representatives and volunteers responsible for the behavior and or medical condition of any of the foster animals or for any damages or injuries to persons or property caused by animals in my care.

I authorize MLD to contact any person or entity named in this application to verify information given. A MLD representative may visit my home before this application is approved or during the time I am fostering a dog for MLD. I understand that MLD will not be liable for any injury or damage to persons or things caused by a foster dog in my care and I agree to indemnify and hold MLD harmless from any such injury or damage.
