• If student fails to adhere to the scheduled payment plan the school reserves the right, without written notice to prohibit student from attending class until unpaid balance is satisfied.
• Any applicant not accepted into this school will be due a complete refund of any fees paid.  This school reserves the right not to accept students they feel cannot benefit from this training.  Students may cancel this contract, in writting within three business days of signing, and will receive a full refund.
• Students who fail to complete this program in the prescribed amount of time will still be charged in full for the course, unless they were absent with a written doctor's excuse.  Students who are absent more than five days, without a doctor's excuse, may be terminated and charged in full for the course.
• The school will charge a registration fee for students transferring to the school and a reentry fee for students who have withdrawn and wish to reenter more than 30 days after termination, of $100.00.
• Students must maintain an average grade of 75, complete all practical service requirements and complete all lessons before recommendations for graduation, state exams or employment can be made.  Pampered Beauty School of Nails will not release, to any licensing board or other schools any certified hours or transcripts unless financial and contractual obligations for those hours obtained have been met.  Transcripts will only be provided for hours that Pampered Beauty School of Nails have been compensated for at the time of request.

I, being enrolled in this course, agrees to pay the fees as prescribed above, to maintain a professional attitude, to obey the state and school regulations, and maintain good attendance while enrolled in this program.  I agree that I am at least 17 years of age with a high school diploma, transcript or GED equivalent.  I also agree that I am a legal US citizen and not involved with crimes related to drug or moral turpitude.

I have read this contract and I understand it is a binding contract.  In the event of unforseen circumstances, the terms of this agreement are covered by SuretyBonds.com.

I understand that I still need to fill out the original contract in office.
