Thank you for your interest in our aesthetic procedures during this unprecedented time of COVID-19.
As a matter of transparency, we are providing information regarding Sars-CoV2, Covid-19 and its relationship to aesthetic procedures here at Allied Medical Aesthetics.
At this time, there is evidence that SarsCoV2 is transmitted via respiratory droplets, breathing and physical contact between people. With that being said, anyone with a fever, cough or shortness of breath will be asked to not enter our establishment and will be encouraged to seek medical attention from an appropriate health care provider. This request applies to all patients, staff & providers of Allied Medical Aesthetics. We will continue to follow ongoing and current Colorado/CDC guidelines regarding sickness, work and isolation/quarantine and will continue to adapt & change our office policies as necessary.
Since we are an aesthetic practice, our providers need to be in close contact with our patients. It is impossible to complete our treatments/services and maintain any sort of the social distancing guidelines. Therefore, limited proximity of such services/treatments, places both the patient and provider at mutual risk for transmission. With that said, Allied Medical Aesthetics will follow CDC regulatory guidelines, DORA & the State of Colorado to help mitigate & reduce any transmission of risks as much as possible. We will regularly clean all surfaces with antiviral cleaning solutions. We will also routinely wear masks, eye shields and gloves to protect our staff members/providers from breathing directly on our patients. Our staff members/providers may also use antiseptic hand sanitizer on top of their gloves to reduce the viral burden even more. As we’ve all been asked to do in social settings per Governor Polis and the State of Colorado, we will encourage all of our patients to arrive & enter our office wearing a mask. If you don’t have one, Allied Medical Aesthetics will provide one for you. A sanitization station will be at the door upon entering. Please be sure to sanitize your hands as you enter and take a disposable mask & pair of gloves if you don’t already have your own.
Even with the precautions listed above, it is impossible to fully guarantee none of staff members/providers and/or patients would not be at risk of potentially acquiring COVID-19. Despite our best efforts to maintain sterilization & cleanliness, plus abiding by all regulatory procedural techniques during treatments, the possibility is still there due to the unavoidable dose of human contact during treatments.
Therefore, as a patient of Allied Medical Aesthetics who is voluntarily willing to receive aesthetic services during this unprecedented time, you are acknowledging to, and have been informed of, the risk for the potential acquisition of COVID-19. Furthermore, you agree to hold Allied Medical Aesthetics, its owners, any staff and/or providers, its landlord, its Medical Director (Michael Iannotti, MD), and all other third parties, other than yourself, harmless for your elective procedure at Allied Medical Aesthetics.
Should you not agree to such terms and/or not sign this consent acknowledging your understanding, you will not be able to receive your service with Allied Medical Aesthetics.
We thank you in advance for your understanding.
Respectfully, Amy Beth & the Team of Allied Medical Aesthetics
