Chapter Meeting Request Form
Chapter leadership my use this form to submit requests to host a chapter meeting. Make sure to fill out the form in it's entirety; missing information will result in the delay of event advertising and lower attendance. If you would like changes made to your request after submission, please email FALA's Chapter Manager at kiki@fala.org. **MEETING REQUESTS MUST BE SUBMITTED AT LEAST 3 WEEKS (21 DAYS) IN ADVANCE OF THE EVENT DATE.**
