TO INSURE THE PROTECTION OF YOUR PET AND OF OTHERS, WE NEED PROOF THAT ALL VACCINATIONS ARE UP-TO-DATE. IF PROOF IS NOT PROVIDED AT THE TIME OF BOARDING, BATH AND GROOMING, VACCINATIONS WILL BE DONE AT THE TIME OF BOARDING BY THIS HOSPITAL.

One of the advantages of boarding, bathing, and grooming at a veterinary hospital is if an emergency arises, medical attention is readily available. If your pet becomes ill, we will contact you and discuss treatment options and an estimate of additional cost. HOWEVER, if you cannot be reached, please indicate your wishes below.

In the event of an emergency and no one can be reached: I authorize the following amount in medical care until someone can be reached.
