Do you acknowledge that if the adopted dog/puppy is not already sterilized, you will have this done by the age of 6 months (or within one month of adopting if the dog is already 6 months).
Do you acknowledge that the full adoption donation must be paid before you take the dog home with you (or before we transport a dog out of area). If after 3 days you find the dog is not a good fit for you we will refund minus the $100 non-refundable deposit. For out of area adoptions, additional travel fees may be non-refundable as well. (note: all adoption fees are tax-deductible)

Do you certify that nobody living in the home has been convicted of animal cruelty, neglect or abandonment?
All of the information I have given is true and complete. This dog will reside in my home as my pet. I will provide it with quality dog food, plenty of fresh water, indoor shelter, affection, annual physical exams and vaccinations under the supervision of a licensed veterinarian.
