Quality and service after the sale is a priority here at Reed Optical. Unfortunately, your Eyemed Vision Plan has their own policies, limiting our ability to provide some of these services. We are required to disclose and follow their policies when glasses are supplied by your plan. Their policies are:

1. Warranty - No scratch or damage coverage unless aadditional coatings such as TD2 scratch or premium non-glare treatment, which provide their own scratch warranty are purchased.
2. Redo - Eyemed will NOT redo lenses for ANY reason, including dis-satisfaction with frame style, materials or coatings.
3. Returns - No returns for any reason after the order is submitted to Eyemed.

I hereby authorize this vision care provider to apply for benefits (order glasses) on my behalf for covered services rendered by them. I also assign my benefits and request that all payments from EyeMed Vision Care be made directly to the vision care provider. I agree that my payment is due in full prior to ordering materials from Eyemed.

I certify that the information I have reported with regard to my coverage is correct. I further authorize my vision care provider to release to EyeMed and its agents any information related to this or any related claim.
