Completed application form and a copy of student transcripts.
Optional: One letter of support from a teacher; one letter of support from an advisor, coach, clergy member, administrator, or community member in support of applicant’s general worthiness of scholarship aid.
Application Deadline: Friday, March 20, 2026, at 3:00 p.m.
When reviewing your application, we want to get to know you as well as we can. There is a limit to what grades and test scores can tell us so we ask you to answer a series of short answer essays. Use this opportunity to introduce yourself to the Awards Committee. Be open and honest. What you tell us in your essays allows us to better understand the information you have provided in your application.
The Sultan F. Khan Memorial Award was established by the colleagues, patients, and friends of Dr. Sultan F. Khan in 2011. A $500.00 scholarship award will be presented to the student who has declared a major which will prepare them for entry into the medical field or who plans to have a career in the healthcare field. Applicants must demonstrate financial need, commitment to the community, and academic excellence.

Applications will be reviewed in early April, and applicants will be notified shortly after. If selected for this award, the Eagle Foundation Academic Awards Banquet will be held on the evening of Wednesday, April 29, 2026. More details to follow.
