    I hereby request BVU Authority (BVU) bill me in accordance with the levelized billing program covered by the Customer Service Policy: Payment Arrangements (Policy) and the most recently-available version of the BVU Authority Rules & Regulations. Levelized billing payments will begin with the next regularly-scheduled Meter Read Date and will continue thereafter until 30 days’ notice of termination has been provided to BVU’s Customer Service department. This plan is payment for all utility services from BVU but does not include any loan amounts due upon the account. I understand there are to be no pre-payments or partial payments and my monthly payment must be made on or before the Non-Pay Disconnect Date. Any termination of service will result in any accumulated balance to be added to the current bill and due immediately. Program termination may limit my eligibility for program reentry in accordance with the Policy.
