Camper/Parent Waiver Form
In an effort to provide a safe and healthy environment for our campers and counselors, Iowa Regular Baptist Camp (IRBC) has implemented the following procedures/policy:
Pre-Arrival Screening:
- All campers must be pre-screened at home, by having their temperature taken, prior to leaving for camp Monday morning.
- Campers may not attend camp if they have symptoms of any communicable disease.
- Past 24 Hours: No vomiting, diarrhea, nasal drainage and/or coughing/sneezing, or a rash
- Past 14 Days: No fever of 100 degrees+ (without aid of fever reducing meds), cough, or shortness of breath
Pre-Screening Questions (you will be asked these questions upon arrival at IRBC):
- Have you been in contact with someone who has COVID-19 in the last 14 days?
- Have you traveled outside of the USA in the last 14 days?
- Do you live in or have you visited an area that is considered a hotspot in the last 14 days?
*If the camper has symptoms or can answer yes to any of the above questions, you must contact IRBC for approval to attend.
Screening Process:
Upon arrival at IRBC all campers will go through a health screening by IRBC staff that includes temperature taken and asked about symptoms & the questions listed above.
Policy:
- If a camper arrives at IRBC with communicable disease symptoms, all arriving in the same vehicle may be sent home.
- IRBC reserves the right to not admit a camper who poses a risk to others.
- Communicable illnesses are an inherent risk of large group gatherings and IRBC is not responsible for campers contracting an illness.
- Should an illness occur, the affected individuals will be isolated as much as possible, parent/guardian will be contacted, and they will be sent home.
Medication:
All medications must be turned into the nurse. It must be in the original container. Please do not send general OTC (tylenol, advil, etc.) if needed, IRBC will provide them.
Insurance:
IRBC has a secondary insurance policy with coverage up to a $5,000 limit, illness is not covered.
Parent/Guardian Acknowledgement:
"I, as a parent/guardian of this camper, have read this policy and understand and agree to the above. I hereby give permission for my child to attend IRBC, to participate in all activities, and for any pictures/testimony of my child to be used in promotion of the camp. I hereby give consent for any medical treatment deemed necessary for my child."
