We have created an online enrollment process and it is a new experience for us. We appreciate your patience and understanding as we may experience some bumps along the way. Please review the questions and directions very carefully as you complete the form. This process has five steps that will go through different staff members for review and completion. You may be contacted by Bellaire staff via email if we need further information or documentation. Please make sure that you check your email daily. There are several very important steps needing to be completed in the process so as to ensure that there is no hold up with your application.

2. Upload ALL documents-you will not be able to complete the process without uploading all of the documents. So please be sure that you gather the following documents before you begin.

3. Name the uploaded documents as directed in the form (for example, Birth certificate, Social Security card, parent/guardian ID, utility bill, lease, immunization records, health records, transcript, report card, withdraw paperwork, school records, 504 paperwork, ARD paperwork, LEP paperwork, etc) The documents do not need the students name because they will be attached to the application.
4. There are two types of enrollments that we are not processing yet.
A.)if you are living with someone else and need a statement of residence form.
B.) if you are not the legal guardian and you need a determination of residence form.
If either of these applies to you, you can complete the form and upload documents. However, we will not be able to complete the enrollment process until the beginning of August.
