Our preferred carrier is UPS. We also offer Fedex for customers who prefer to ship on their account number. *Clients preferring a different carrier may arrange to have goods picked up from our location. We ship FOB Los Angeles, CA 91304

SHIPPING METHOD ( i.e., Ground, Next Day Air, etc)*

Please indicate the specific method based on the carrier we are shipping on. For example, "Priority Next Day Air" is NOT an actual shipping method. Refer to the carrier's website for specific shipping methods.

Shipping on a 3rd Party Account?

By Shipping 3rd Party, you are authorizing us to use your shipper account, or your client's account as an authorized agent of your client and assume liability for any charges associated with chargebacks or billing issues per our Terms below.

I authorize Lady Fortunes Inc to bill the following shipper number UPS or FedEx # only for shipment for my PO/Order# order number .
The account belongs to and the billing address associated with the account is Street Address Address Line 2 City State Zip .
If the account belongs to my client, I am asserting that I have authority to use their third party account as an agent of my client. I am responsible for any charges incurred that my client refuses to pay.

submit a separate address list per each shipping method. Make sure to SORT list that has DIFFERENT QUANTITIES in order of smallest to largest. We are not responsible for errors made if we are modifying your Excel list.

DROPSHIP TERMS & CONDITIONS

Orders requiring multiple destinations require addresses to be formatted on our DropShip Form. Excel Spreadsheets will not be accepted unless they are in our DropShip Form format.

Lady Fortunes is not liable for incorrect or undeliverable addresses on DropShip Form. Customers must ensure address fields contain the appropriate amount of characters: fields containing too many characters will be cut off on the shipping label.

Lady Fortunes will not be responsible for any delays or charges stemming from information being cut off. Please note: duplicate addresses will be processed as individual shipments.

We will not group shipments together unless agreed upon in writing.

Shipping labels will be made according to Dropship Form individual line items.

DIFFERENT SHIP METHODS

If you have DIFFERENT. shipping methods ( i.e., Ground and 2 Day Air) please separate your addresses BY shipping method.

DIFFERENT QUANTITIES

If you have DIFFERENT QUANTITIES on your list, you must SORT in order of LEAST TO GREATEST. Otherwise, we are not responsible for errors made due to sorting issues.

INCORRECT/UNDELIVERABLE ADDRESSES

Incorrect/undeliverable addresses will be subject to billing additional fees including Address Correction Fees, Return Fees, Re-Route Fees, Administrative Fees, etc. This is regardless of whose shipper# shipment was sent on as these charges are billed to shipper- not account holder.

By Shipping 3rd Party, you are authorizing Lady Fortunes to use your shipper account, or your client's account as an authorized agent of your client. You will be liable to Lady Fortunes for any chargebacks associated with the shipment. This includes undeliverable/incomplete/incorrect address fees (billed to Lady Fortunes regardless of which account is used) and refused or unpaid charges which are billed back to Lady Fortunes.

Lady Fortunes charges an administrative fee of $15 per shipment charge on top of carrier fees for any address corrections or rebills.

The minimum charge for address correction is $35.
