  • Feline Exam Form

  • Due to COVID-19, we are only offering curbside services. To help the check-in flow and to minimize wait time, please complete this form prior to your arrival.

We have installed teller-style windows located on either side of our lobby door. Check-in is to the right of the door, check-out is to the left. Upon arrival, we ask that you come to the check-in window. Once checked-in, you will be asked to return to your car until the vet is ready. The assistant will call the cell phone number provided to review your pets visit details. When finished, they will ask you to meet them at the door. Once inside, the vet will examine your pet. Once complete, the vet will call you to review the findings and confirm treatment plan. Once the exam is complete, the vet or assistant will instruct you to come to the front for check-out. Your assistant will then bring you your pet along with any items being sent home.

  • Rabies Vaccine

All patients 16 weeks or older MUST have an up-to-date Rabies vaccine as required by state law. If your pet does not have a current Rabies vaccine, one will be given in the office. *Rabies should be given at 16 weeks old, then every 3 years for life.
  • FVRCP Combination Vaccine ($28)

This vaccine protects against 3 common and potentially life threatening diseases: Rhinotracheitis, Calicivirus, and Panleukopenia. These viruses are highly infectious causing respiratory disease, mouth sores, and diseases of the gastrointestinal tract, immune system, and nervous system. Cats can contract these diseases through direct contact with other infected cats, contaminated objects, and through the virus persisiting in the environment. *FVRCP should be given at 8,12, and 16 weeks of age, 16 months of age, then tri-annually.*
  • Leukemia Vaccine ($28)

Leukemia is a virus shed in many bodily fluids, including saliva, nasal secretions, urine, feces and blood. It is most commonly transmitted through direct contact, mutual grooming and through sharing litter boxes, food and water bowls. It can also be passed in utero or through mother’s milk. Outdoor cats who get into fights with other cats can transmit the disease through bites and scratches*Booster after 30 days, then annually.*
  • Felv/Fiv Combo Test ($39)

FeLV that is currently affecting the cat is fatal. Some cats can be carriers, but not sick from the disease yet. They should be isolated from other cats indefinitely (especially if they fight or mate). FIV is similar to HIV in people. In general, these cats are isolated to prevent the spread. They have a compromised immune system, so it is easier for them to acquire illness. *Testing is recommended as early as 10 weeks of age.
  • Fecal Parasite Testing ($20 or complimentary with exam)

Fecal examinations are primarily performed to detect microscopic gastrointestinal parasites, such as roundworms, hookworms, whipworms, Coccidia and tapeworms.
