    • I hereby give my approval for my child’s participation in any and all activities prepared by Oku Education. In exchange for the acceptance of said child(ren)’s candidacy by Oku Education, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Oku Education and all its respective officers, agents, and representatives from any and all liability for injuries to said child(ren) arising out of traveling to, participating in, or returning from selected activities.

      In case of injury to said child(ren), I hereby waive all claims against Oku Education, including all teachers, aides, staff, volunteers and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all activities. 

    • Medical Release and Authorization

    • As Parent/Guardian of the named child(ren), I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child(ren), in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

      Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me, or any listed emergency contacts, in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

      Permission is also granted to Oku Education and its affiliates including Directors, Teachers, Aides, Staff, and Volunteer Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

      Release authorized on the dates and/or duration of the registered term/year.

      This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child(ren), in my absence.

    • Liability

    • I am a parent or legal guardian of the registered who is currently under the age of 18, grant permission for the registered child(ren) to attend Oku Education and consent to its Discipline Policy and also do hereby release Oku Education from any and all liability or responsibility due to any injury that he/she/I may incur as the result of, or arising in any way from participation in activities at or under the direction of Oku Education. I am fully aware and consent to accept these risks and voluntarily agree to allow his/her participation in activities. If I cannot be consulted in an emergency, I hereby give permission to the physician selected by a representative of Oku Education to hospitalize, secure treatment for, and to order injections, anesthesia, and/or surgery for the person named above. I understand that Oku Education carries insurance but that I will take primary responsibility for any fees or charges at any clinic, facility, or hospital arising from treatment of injury or illness.

    • Media Release

    • Oku Education requests your permission to reproduce through printed, audio, visual, and/or electronic means activities in which your student has participated in his/her educational program. Your authorization will enable Oku to use specially prepared materials to train teachers and/or increase public awareness and promote continuation and improvement of education programs through the use of mass media (i.e., Instagram, Facebook, other social media sites), displays, brochures, and/or websites, etc. In addition, during class time, Oku may take candid photographs, video, and/or audio recordings and your student may have his/her photograph, name, audio recording and/or video in a publication (i.e. newsletters, informational handouts) or website. Parents or guardians may opt-out of having their students’ photograph or video recording used in Oku materials including newsletters, brochures, websites and video, used on the Internet or in other venues by completing this form. Students, parents and visitors should be aware that Oku does not have the legal authority to prevent your student from being photographed or filmed by news media or other parties at public events such as field trips or student performances. I understand that Oku Education often takes photographs or videos of students during its classes, activities or events. Oku Education will not identify me or my student(s) by name, or release any other personal information without additional written permission from me.

      This policy must be filed with Oku each school year and remains valid through the entire school year. Additionally, I agree that I shall have no right, title, or interest in these published images or recordings, nor shall I find cause for action or injuries reported from them. I understand that I will not receive monetary compensation for any publication herein authorized.

    • Discipline

    • Teachers, Aides, Staff and Volunteers are facilitators/mediators when it comes to conflict resolution. They give children the tools that will help them work and solve their problems. If the problem persists, and the children cannot resolve the conflict themselves, then facilitators will help them solve the problem.

       

      Discipline Policy:  Oku Education believes in positive behavioral approach discipline.

      • Redirection to another area or activity
      • Talking with the child(ren) about the problem
      • Keeping the child(ren) with a facilitator
      • Having the child(ren) help with a specific task or duty.
      • Allowing child(ren) to work through the issue

       

      Incident reports must be filled out for a child causing injury to another intentionally.  The name of the aggressor will not be disclosed to other parents by staff.

      • Following an incident report, the staff will work with parent/guardian to come up with solutions to ensure the behavior is not repeated.
      • If a child continues to consistently act out after parents and staff have implemented the plan, the director will be consulted for further action and a new behavior plan will be put into place.
      • If the aggressive behavior continues to escalate and a solution cannot be found parent/guardian will be notified and child’s enrollment can be discontinued.

       

      The basic rules are:

      • Keep hands and feet to oneself.
      • Follow the direction of adults respectfully.
      • Use equipment properly.
      • Treat other children and adults with courtesy.
      • Use appropriate language suitable for a school setting.
      • Settle disagreements by discussing them. To ensure a safe, courteous, and positive environment, it is necessary to deal with unacceptable behavior firmly.  
      • A disruptive child may be removed from a group situation to regain self-control.
    • Off Site/Field Trip Authorization

    • I, the parent/guardian of the child(ren) named above, hereby give permission for my child(ren) to take part in field trips arranged by Oku Education. I understand that the following conditions apply:

      1. I understand that I am responsible for getting my child(ren) to and from the departure and return sites. I understand that my child(ren) shall be accompanied by staff member(s) during the trip.
      2. I understand that my child(ren) is expected to behave responsibly and to follow the discipline policy.
      3. I agree and understand that I am responsible for the actions of my child(ren), and I release Oku Education from all claims and liabilities that arise in connection with the field trips, except if due to the negligence of school officials.
      4. I confirm that my child(ren) is medically fit and able to participate in all activities
      5. I have given notification to Oku Education staff of any permanent or temporary medical or other condition(s) including special dietary and medication needs, or the need for visual or auditory aids, which should be known about my child(ren).
      6. I understand that as a parent, if I believe it is necessary to limit my child(ren)’s activity to a great extent, that Oku Education may not be able to accommodate my child(ren) on a field trip and that I and my child(ren) will be informed of this decision as soon as possible.
      7. I agree that in the event of an emergency injury or illness, the staff member(s) in charge of the trip may act on my behalf and at my expense in obtaining medical treatment for my child(ren).
      8. I understand that students who violate Oku Education’s discipline policy may be excluded in the future by the staff from participating in a field trip.
      9. I understand that as an adult volunteer I am present to attend to my child(ren), and to any child(ren) in my care that are not registered. I understand that all actions of a non-enrolled child(ren) are my sole responsibility and if a non-enrolled child(ren) cannot follow the discipline policy or behave responsibly that prompt removal may be requested by Oku Education staff or volunteers to ensure the safety of all other child(ren).
      10. I understand that fees incurred for a field trip for non-enrolled students and parent/guardian attending are not covered by Oku Education, and are my responsibility.
    • Parent/Guardian Conduct

    • While in the presence of children and/or Oku Education teachers, aides, staff and volunteers, parents are expected to maintain a professional demeanor. Other than their own children, parents may not at any time approach children in the program for discipline reasons. All behavioral and disciplinary concerns must be directed to the staff. Any parent/guardian, or other person who insults or abuses any teacher, aide, staff or volunteer in the presence of other school personnel or children at a place which is on school premises or public sidewalks, streets, or other public situations in connection with assigned program activities is unacceptable and may result in a request to leave.  If the situation escalates and Oku Education teachers, aides, staff, or volunteers feel threatened, local authorities will be notified.  If parent/guardian has repeat offenses this may require further action by the Oku Education director, which may result in termination of enrollment if a solution cannot be found.

    • Illness

    • Your Child Must Remain Home If

      • Fever (Oral: 100.4+, Rectal: 101.4+, Axillary: 99.4+) – May return 24 hours after fever has subsided and temperature returns to normal without the aid of medicine.
      • Colds/Flu/Infectious rhinitis, i.e. colored nasal discharge or nasal congestion associated with a cough and fever. May return as soon as thick yellow or green mucous discharge is no longer draining from the nose. Coughing subsides.
      • Vomiting, Diarrhea/change in: consistency, frequency, color or odor of stools. May return when loose stools have subsided and the child has returned to normal eating without stomach upset.
      • Red Watery Eyes. May return when eyes are no longer burning or itching. Conjunctivitis, such as pink eye or thick discolored drainage Symptom free for at least 24 hours.
      • May return when rash disappears or is determined that rash is not a communicable disease. Physician’s/Health care professional’s note is required upon re-entering.
      • Parasites (i.e. nits, crabs, lice, etc.) Symptom free for at least 24 hours. Must provide clean check proof from a trained professional (i.e. Picky Pam’s, your pediatrician, health care professional, etc.)

       

      All contagious illnesses require a physician’s or health care professional’s clearance to return to classes for the safety of all children.

       

      Oku Education does not have facilities for sick children. Review the symptom chart to determine when your child(ren) must stay home.

       

      If your child becomes ill, he/she will be isolated as much as possible and must be picked up WITHIN ONE HOUR.

       

      Please notify Oku Education if child(ren) is/are ill and unable to attend.  It is absolutely necessary that Oku Education be informed if your child(ren) has any contagious diseases such as chicken pox, measles, mumps, head lice, etc.  Also please inform Oku Education if your child(ren) has been exposed to any contagious illnesses so that we can be alerted to early symptoms. We will notify all parents when a child has been exposed to a communicable disease.  ALL notifications will be anonymous of child or family to the community by staff.  Anonymity will extend to all staff as well if notification is only given to admin.  These requirements are legally protected.  Please do not ask admin, staff, volunteers, etc. to name child(ren) or family as this information will not be shared and in many cases may not be known.

       

      Medicine will not be dispensed by faculty or volunteers unless they are deemed necessary for life threatening situations, such as Epi-pens, with clearance from parents.

       

      On site first aid is limited to band aids, and ice packs.

    • Device Policy - Cell Phones and Two way devices

    • We recognize that devices have become a common tool for communication.  However, they have also become a major distraction to the learning environment.  We ask that you leave the devices that are not absolutely necessary at home.  Students are allowed to have devices at school, but they must remain stored out of sight in backpacks until they leave campus.  Use of devices during program day to talk, take pictures, play games, record or text is strictly prohibited.  If a student violates the device policy, the device will be confiscated and turned into administration to keep for the duration of the day.  Parents or responsible adult may come up to claim the device at the end of the day, the child may not.

      Oku Education is not responsible for lost, damaged or stolen devices, remember most backpacks are stored outside, devices will be unattended.

    • Drop/Withdrawal Policy

    • We have a 30 day drop/withdrawal policy, written notification must be received at oku@okueducation.org for the 30 days to begin, and a full month of program fees will be due at time of drop/withdrawal.

      Written notification must be in the form of an email to oku@okueducation.org, and a confirmation will be sent within 1 to 2 business days acknowledging receipt of notification.  The 30 days will begin from the time of submission of the email.  At the conclusion of the 30 days, registration of child(ren) will be void.

      The month of program fees will be charged to the guardian or responsible party for the drop/withdrawal notice timeframe, this will be in addition to any program fees that are being paid for services rendered. 

      Example: If I withdraw on the 15th of the month, my child(ren) have attended through the 15th, my 30 days notice starts on the 16th, and I am being charged the month of program fees from that point forward.  I owe for the 1st-15th for services rendered, and my drop period of payment will run from the 16th for 30 days.

      Guardian or responsible party will receive an invoice within 5 business days of drop/withdrawal notification.  Charter funds may be used to pay the amount or partial amount due, but the cancellation of the PO will result in an invoice being sent immediately to the guardian or responsible party for payment upon receipt. 

      Registered child(ren) may still attend during the 30 days if desired.

       

    • BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
