The information provided on this form will be used by the SYNC Athlete Fund Committee to select recipients for grants. It is important that you give complete details concerning your athletic career plans, activities and financial information. Your information is fully confidential and secure.
The goal of the SYNC Athlete Fund is to provide financial support to athletes with potential to be elite who have demonstrated financial need and would not otherwise be able to afford basic necessities like coaching, travel and equipment to achieve even higher performance levels and rankings. Just as important as athletic success, the opportunity to compete and attain the highest achievement in sport prepares athletes for leadership roles now and in the future and provides a platform to serve as role models for their peer athletes and communities.

Application Eligibility: For an athlete to be considered, he/she must:
1. Athletes from all nations are eligible to apply
2. Have amateur status but have a successful competitive record within your age group.
3. Not be a fully funded member of a national team
4. Demonstrate financial need. The SAF approves grants that will have a significant lasting impact on the applicant. A request needs to demonstrate visible, apparent, and immediate financial need and be consistent with the Fund’s mission.

VI. Agreement
Thank you for applying to receive a grant from the SYNC Athlete Fund. Please review the Agreement below and verify your understanding and commitment to the requirements of the Agreement by acknowledging and signing this form. By signing this form, and receiving the SYNC Athlete Fund grant funds, this Agreement is a contract between you and the SYNC Athlete Fund.

1. COMMITMENT TO DEVELOPMENT AND SPORTSMANSHIP. Athlete agrees to dedicate his/her efforts to personal development and support of alpine ski racing in general. Athlete will actively promote and role model good sportsmanship, the importance of leading a healthy lifestyle and strong school performance, and the avoidance of substance abuse, including abstinence from the use of performance enhancing drugs.

2. GRANT USES

2.1 Allowable Expenses. Athlete shall use Grant Funds only for the following expenses: (a) training costs, (b) coaching, (c) travel to competitions (d) competition fees, (e) housing, (f) equipment.

2.2 Accounting. Athlete shall cooperate with any requests by the Foundation to verify that Athlete has used the Grant Funds in accordance with this Agreement. Such requests may include but are not limited to, receipts, invoices, evidence of payment, and such other documentation sufficient to confirm that the Grant Funds have been used for Allowable Expenses.

3. TAX REPORTING RESPONSIBILITY.

Upon receipt of the Grant Funds, Athlete assumes "paymaster" responsibility of such funds, including responsibility for tax reporting and related obligations imposed by law upon a recipient of such funds.

4. TERMINATION AND GRANT REVOCATION

4.1 Misuse of Grant Funds. The foundation may terminate this Agreement immediately upon a determination, in its sole discretion, that the Athlete has violated Sections 2.1 or 2.2. Upon such termination, Athlete shall be required, and hereby agrees, to return and fully refund the Grant Funds to the Foundation.

4.2 School or Athletic Sanction. If the athlete for any reason is subject to a sanction or fails to remain in good standing with his/her school or athletic organization, the Foundation may terminate this Agreement in its sole discretion. Upon such termination, Athlete shall be required, and hereby agrees, to return and fully refund the Grant Funds to the Foundation.
I certify that all of the information that I have provided to SYNC Athlete Fund is true and correct. I also certify and represent that I am drug free and am not currently the subject of a school or athletic sanction. Additionally, I hereby authorize the SYNC Athlete Fund and TA Foundation Grant Committee to verify the information, statements and representations made in this application. I also authorize SYNC Athlete Fund to contact third parties to verify the information that I have provided herein.
