The contract is made duplicate and entered into for the weeks of September 5, 2023, through June 13, 2024, between “Grant’s Chesapeake Taekwondo and After School Center” and the undersigned parent/guardian of the above child's name

TRANSPORTATION

The Chesapeake Public Schools provide transportation for Butts Road Primary, Butts Road Intermediate, and Southeastern Elementary Transportation from all other selected Chesapeake Public is provided by the center. Parents must notify the center at least 2 hours in advance should a child not need to pick up on a specific day due to illness, parent pick-up, early dismissal, etc. A “NO SHOW” charge of $25.00 per incident will incur if an employee of Grant’s Chesapeake Taekwondo and After School Center is not notified before arriving at a school for a pick-up. On early release days, our center will be open two hours early at no extra cost.

 

I/We agree to make payments per week as stated above and an $80 registration fee to Grant’s Chesapeake Taekwondo and After School Center (GCTKD&ASC). If I do not pay the weekly payment within 30 days of the date on which it is due or breach any other terms of this agreement, GCTKD&ASC may demand the entire unpaid balance be paid immediately and proceed in the collection of this amount. I agree to pay the cost of collection, including Attorney’s Fees.

 

Being a member of GCTKD&ASC, I will faithfully comply with all of the rules and regulations of the center and the traditions of Taekwondo. Students understand that during the course of instruction, employees of GCTKDD&ASC and/or other students or authorized persons will engage in a course of conduct requiring physical contact, and he/she gives full consent to such contact as required by training.

 

I/We understand and agree that “Grant’s Chesapeake Taekwondo and After School Center” will not be held liable for any inquiries, damages, etc., not caused by or resulting from the negligence of the owners, operators, or person in charge of such establishment, of their agents, servants or employees. I understand further that strict observation by me of the rules and regulations relative to my child/s’ training, including the use of protective equipment required by GCTKD&ASC will largely eliminate the possibility of accident or injury and that I waive any claim of damages against GCTKD&ASC and/or its principles or instructors in any case resulting from the activity.

***Please read the following financial contract carefully. It explains absence policies, snow days, teacher workdays, school being closed, Christmas and Spring Break, NO SHOW and Pandemic***

 

1.       Parent(s)/guardian to pay the agreed amount below per week no later than Sunday of each week. If payment is made later than Tuesday, an additional $20.00 late fee will be paid. When schools are closed due to holidays, teacher workdays, etc., there will be a $30.00 all-day fee on top of that week’s tuition, if you wish to use the facility. Pick-up time is by 6:00 PM each day, after 3 late incidents, a late fee of $1.00 per minute will apply starting at 6:01 PM.

 

2.       A non-refundable fee of $80.00 per child for Registration is required upon sign up.

 

3.       A “NO SHOW” charge of $20.00 per incident will incur if an employee of Grant’s Chesapeake Taekwondo and After School Center is not notified prior to arriving at a school for a pick-up.

 

4.   The full week’s tuition will apply to all other weeks included in the Chesapeake Public School District Calendar regardless of snow days, teacher workdays, absences, holidays during the week, Christmas, Spring Break, and in the event of a Pandemic. Our program is sold per school year (not by week.); the payment is divided weekly based on the number of weeks every school year.

 

5.       We do not charge for days during early release.

 

6.       The parent/guardian agrees to pay a $20.00 charge for any declined credit/debit card on file. The After-School Program reserves the right to require having a card on file upon registration.

 

7.       Additional Fee (if applicable) Testing Fee and Tournament Events.

 

The parent/guardian agrees to notify The After School Program’s main office four weeks in advance of the removal of their child from the program. Parents/guardians will be responsible for the weeks' tuition if advanced notification is not given.
