I agree to the following terms and conditions:

I agree to always walking my foster dog wearing both a collar and a harness with 2 leads, one secured on each. I will never let the dog run off-lead or let outdoors without supervision.
I agree that the dog is being fostered by myself and will not be sold, adopted, or given to another party.
I agree that the foster dog must be de-sexed (puppies by 6 months of age), if not spayed/neutered prior to fostering.
I agree to care for the dog in a humane manner. This includes supplying adequate food, water, exercise, attention, and care. The dog is to be a companion animal, not a guard dog and will live inside my home.
I understand and agree that SDC makes no guarantees about the animal's temperament and is not responsible for future damages or injuries caused by the animal.
I will inform SDC immediately if the dog is lost, hurt or dies.
I agree to hand over the dog to SDC once my foster period is over. Failure/refusal to hand over a dog to the rescuer will lead to legal action being taken.
I agree to going through the proper adoption process if I would want to keep the dog. Otherwise the dog will remain under the care of SDC and can be adopted by someone else.
I give SDC permission to call or visit my home at any reasonable time to assure that the dog is being properly treated and cared for. I will also give regular updates on how the dog is doing.
I agree that if at any point I cannot keep the dog, I will return him/her to SDC without requesting any compensation.
I agree to keep SDC informed of my current home address and phone number during the foster period.
SDC has the right to remove the foster dog if they feel the environment is considered to be unsafe, unloving or unhealthy for the dog. I shall be held responsible for the costs incurred due to the necessitated removal of the dog. This shall include any veterinary costs related to the reason/s for the removal of this dog.
I confirm that all statements I have made on this form are true. If it is found that anything I stated on this form is not true SDC can remove the dog from my care.
