  • The Agreement

  • By submitting this form I agree to the following terms and conditions:

    •    I agree that the animal is being fostered by myself and will not be sold, adopted, or given to another party.

    •    I agree that the animal will not be allowed outdoors without supervision.

    •    I agree when taking my foster dog outdoors, s/he will be on a secure harness/leash and wearing proper ID.

    •    I agree that this foster dog is to be a companion animal, not a guard dog. The dog will live inside my home. 

    •    I agree that the foster dog must be de-sexed by 6 months of age. If the animal is not spayed/neutered prior to fostering, I will contact SDC UAQ to arrange for my foster dog to be de-sexed at 6 month of age.

    •    I agree to care for the animal in a humane manner. This includes supplying adequate food, water, shelter, attention, and medical care. 

    •    I agree that if at any point I cannot keep the animal, I will return him/her to the rescuer without requesting a fee back. 

    •    I understand and agree that the rescuer makes no guarantees about the animal's temperament and is not responsible for future damages or injuries caused by the animal. 

    •    I give the rescuer permission to call or visit my home at any reasonable time to assure that the dog is being properly treated and cared for.  

    •    The rescuer will have the right to remove a puppy/dog if they feel the environment is considered to be unsafe, unloving or unhealthy for the puppy/dog. The adopter shall be held responsible for the costs incurred due to the necessitated removal of the puppy/dog. This shall include any veterinary costs related to the reason/s for the removal of this puppy/dog.

    •    If dog is lost or dies, the foster is obliged to inform the rescuer.

    •    I agree to hand over the pet to the rescuer once my foster period is over. (failure/refusal to hand over a dog to the rescuer will lead to legal action being taken)

    •    I confirm that the foster dog will remain under the care of SDC UAQ and I will not try to keep the dog without going through the proper adoption process.

    •    I agree to keep the rescuer informed of my current home address and phone number. 

    •    I agree that all statements I have made on this form are true. If it is found that any statements I have made on this form are not true the adopted animal can be confiscated.

    •    In case of reasons beyond my control I agree to give a notice of 15 days to return or relocate the dog to the rescuer.
