Terms and Conditions
1) I fully accept the risk involved with the ministries of and hold harmless from any legal liability, Connersville Baptist Temple and any persons involved in these ministries.
2) In the event of an emergency that requires medical treatment for the above named child, I understand every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give my persmission for CBT volunteers to secure the services of a licensed physician to provide the care necessary for my child's wellbeing. I assume responsibility for all costs connected to any accident or treatment of my child.
3) I grant my permission for any photos and/or videos of my child to appear in any church publication, slide presentation, website, social media post, or bulletin board display at CBT.
4) I grant permission for my child to travel to/from CBT events with an adult leader.
5) I am responsible for updating CBT with any changes in information.
