AA-CyFair-Music (AA/FOS) Drama Student Guidance Form

This form will enable the Chair of Performing Arts and Departmental Faculty to guide students toward graduation and transfer success goals. The form should be completed by all AA/FOS students in the Performing Arts at the beginning and middle of each semester. We encourage other students participating in drama courses and productions to fill out this form, as well. If students do not wish to complete one of the fields, they may simply mark “n/a.” The more information we have, the better we can serve our students.
