Upload BOTH your signed Form 13615 (Volunteer Agreement, not the certificates) and your valid photo ID below.(After you pass the exams, sign your Volunteer Agreement in Link & Learn and then download the pdf file of your signed 13615 to your computer. Upload that pdf and a scan or photo of your ID if we don't have it this year. These must be renewed each year.
