MODEL RELEASE

I hereby give permission to SUNY Oswego and its assignees to use my name (or any fictional name) and photographic likeness and/or footage in all forms and media for advertising, trade, and any other lawful purposes. I waive the right to inspect or approve the finished version(s)—use of the imagery and written copy or finished products—that may be created in connection therewithin.

I grant to SUNY Oswego the permission in perpetuity to use the imagery, either wholly or in part, in advertising, websites, blogs, social media, print media, display, portfolios, or any other promotional methods for SUNY Oswego with any retouching or alteration without restriction.

I will not prosecute or institute proceedings, claims or demands against SUNY Oswego in respect to any usage of the above mentioned imagery. I hereby release SUNY Oswego and its assignees from all claims and liability relating to images, footage or photographs taken of me.
