  • As of December 30, 2021,  we have streamlined our Entrance Form.  The previous, more thorough one is on our website, kaplinortho.com.

  • Clear
  •      Dr. Kaplin requires knowledge of all exposures as a condition of entry.  Entry is cleared by Dr. Kaplin, based on his best assessment of all factors.  Other healthcare professionals, such as MDs, school nurses, and the CDC cannot clear any patient for entry.

As of 4-1-2021, until the pandemic has ended, we are asking anyone who has either contracted Covid within the last two weeks, or has been recently exposed, or may possibly have Covid, to postpone their visit for a short period of time.

We have found each situation to be unique.  If we have the facts, we can make our best judgement.  Please trust that we have no agenda other than the safety of all concerned.

  • I represent that these questions are truthful.  I accept that Dr. Kaplin will use his best judgment, and it is possible that I may be asked to reschedule today’s orthodontic appointment.

    Protocol for Visiting Kaplin Orthodontics:

Before you leave for your appointment:

    • We are asking our patients to brush their teeth at home before coming into the office. We have been directed by the ADA (American Dental Association) to close our tooth brushing areas during this time. This includes brushing in the restroom.
    • Please don’t forget to bring whatever type of mask you have at home. It does not have to be a surgical mask; a homemade mask will suffice. If we can spare them we will; but there is a nation-wide shortage.
    • Please have already have filled-out and submitted the online Consent and Health Questionnaire we sent you by text or email. We will have forms here if necessary, but we are trying to minimize paper and pen usage.
    • Please remember to use the bathroom before you leave the house, if you can. We have been instructed to severely limit the usage of our bathrooms.

    When you arrive:

    • TEXT us at 224.548.9862 or CALLus at 847.253.2300 (Arlington Heights) or 847.438.8899 (Lake Zurich) to let us know you are here. We will text or call you when it is time for our patients to come into the office for their appointment.
    • If our patient is under 11 years old, an adult is welcome to come in. But, the adult will also need to come in with a mask (of some sort) and allow us to take their temperature and answer a few questions to ascertain if they are ill,  or potentially ill. For 11 and older, we prefer our patients come in alone.  But a parent will not be turned away if they come in.
    • If there are financial papers to sign, the parent is welcome to visit the front desk, but they are required to wear a mask (of some sort), and allow us to ascertain that they are not ill.
    • The assistant who will be with the patient will greet them at the door. After our patient is deemed healthy, the assistant will escort the patient to the treatment area.

  • In the office:

    • No parent will be permitted to come into the clinic area except if they are under 11 years old.
    • Our patients will be asked to wash their hands both before and after their visit. We will be giving protective eyewear (cute sunglasses) to have on while they are in the office.

    Making the next appointment:

    • For the first visit, or two, our plan is to take the liberty of giving our patients their next appointment date & time. We want to take full advantage that there is no school, sports, band, camp....and many parents are working from home. If the appointment we give you does not work for you, you can call us and we will make you another one.

    Communicating with Dr. Kaplin:

    • By this point in time, imagine how many parents want to know if their child’s orthodontic treatment is on-schedule, behind, are doing ok.....Please send your child in with their cell phone. They can call you in the car and we can catch up on the speakerphone. Or, we can tell your child the info you need to know. Or, Dr. Kaplin can follow up by email, text, phone,
