• Behavior TLC: (CL) Fees & Service Agreement

  • Description of services

    Behavior TLC, Inc. will provide applied behavioral analytic services to the Client and all those involved with the Client’s progress. These services could include several of the following but are not limited to observation or video of Client in various settings, Parent/Guardian discussion, treatment formulation, report preparation, ongoing communication and supervision with outside therapists employed by the Client, and any other types of contact/consultation necessary to provide adequate services.

  • Description of Client

    The Client in the Agreement as stated above.  All behavioral services will be individualized to the Client. Behavior TLC, Inc. is obligated to assess the treatment effects on all parties associated with the Client including Parent/Guardians and therapists.

  • Demographic Information

  • **If Divorced or shared custody, we require a copy of the divorce decree that details who is responsible for medical/educational decisions, and who is financially responsible for medical/educational services.

    Understanding of Progress

    Behavior TLC, Inc. makes no promise or guarantee regarding the outcome of behavior services. Results may vary depending on the Client, the service, the target behavior(s), and adherence to the treatment plan by all involved parties.

    Behavior TLC, Inc. will make necessary attempts at informing the Parent/Guardians of the Client’s progress in the form of phone conversations, face to face meetings, and regular reports.

     

    Statement of Confidentiality

    Behavior TLC, Inc. has a primary obligation and will take reasonable precautions to respect the confidentiality of the Client, recognizing that law, institutional rules, or professional or scientific relationships may establish confidentiality.

    In order to minimize intrusions on privacy, Behavior TLC, Inc. will include only information germane to the purpose for which the communication is made in written and oral reports, consultations, and the like.

    Behavior TLC, Inc. will discuss confidential information obtained in the consulting relationship, or evaluative data concerning the Client, with supervisees only for appropriate scientific or professional purposes and only with persons clearly concerned with such matters.

     

    General Attendance Policy

    Behavior TLC requires that clients attend an average of 90% of their scheduled session time. If a 90% average is not maintained, due to unexcused absences, the following procedure will be enacted.

    Infraction 1: After the first infraction, an improvement plan is put into place (i.e. additional parent training, make-up sessions, etc.).

    Infraction 2: If a second infraction occurs, in a twelve-month period, Behavior TLC reserves the right to discontinue services.

    If at any time a client does not maintain at least a 75% attendance average of scheduled session time, including excused and unexcused absences, the following procedure will be enacted.

    Infraction 1: After the first infraction, an improvement plan is put into place (i.e. additional parent training, make-up sessions, etc.).

    Infraction 2: If a second infraction occurs, in a twelve-month period, Behavior TLC reserves the right to discontinue services.

    Excused absences are considered incidents of nonattendance that have been scheduled at least a week prior to the requested date and approved by administration. Absences that have not been scheduled but are supported through appropriate documentation (e.g. doctor’s note) could also be considered excused. Other examples of excused absences could include observance of religious holidays, required legal obligation, or healthcare appointments. Any absence that does not meet the criteria listed above is considered to me unexcused.

     

    Interruption of Services

    Cancellations and No-shows:

    If unexpected events occur which would interrupt/delay services such as illness, both parties involved in this agreement, Behavior TLC, Inc. and Client, will give appropriate notice to the other party in the form of phone communication or written notice at least 2 hours prior to appointment time. No-shows or less than 2 hours’ notice will incur a cancellation fee of 100% of the scheduled session. If you are an insurance client, your no-show fee will be billed in a cash invoice.

    Late Arrivals:

    The appointment begins at the set appointment time and will be billed as such. If a client is late arriving to the appointment they will be billed for the full appointment time. For insurance clients, a cash bill will be invoiced for the late arrival time and the rest of the session time will continue to be submitted to insurance as usual.  For example, if the appointment begins at 8:30 and the client arrives at 9:00, the client will be billed our cash rate for 8:30-9:00.

    Late Pickups:

    The appointment ends at the set appointment time.  All clients must be picked up at their pre-determined session end time.  Our staff often have back-to-back sessions and need their transition time between clients.  After 5 minutes, there will be a $10 cash late fee charged and an additional $1 for every minute after that. Once the session has ended we will not continue therapy, therefore we will not charge the insurance company for that time. 

    Vacations/Leave of Absence:

    The BTLC calendar offers 5 weeks of closures for families to be able to take vacations without missing any clinic time.  Of course, other instances will come up when a client needs to be away from the clinic.  Behavior TLC cannot hold any client’s session times for longer than a 2 week break.  After 2 weeks of absences (scheduled or not) that session time will be given to the next family on our waiting list.

     

    Termination of Services

    This Agreement will remain in effect until either Behavior TLC, Inc. or the Client notifies the other party of a desire to terminate services.  Such notice must be tendered in writing, either in person, by certified mail, or by email.

    If the Client terminates services, Behavior TLC requires prior notification of 30 days.  This time period allows us to adhere to our ethical obligation of transition services: communicating with the next service provider, collecting relevant stimuli and other necessities to send home, finalizing reports, and conducting appropriate exit interviews with the family.  Behavior TLC cannot guarantee these necessary transition services without the 30 day notice.

    If Behavior TLC, Inc. initiates termination of services, Behavior TLC, Inc. will give 30 days’ notice in writing to the Client. Prior to termination, except where precluded by the Client's conduct, Behavior TLC, Inc. will discuss the Client's views and needs, provide appropriate pre-termination services, suggest alternative service providers as appropriate, and take other reasonable steps to facilitate transfer of responsibility to another provider if the Client needs one immediately. Termination of the professional relationship by Behavior TLC, Inc. will occur when it becomes reasonably clear that the Client no longer needs the service, is not benefiting, or is being harmed by continued service.

     

    Description of Fees

    The following services are billed at an hourly rate: BCBA Consultation, BCaBA Consultation, Club TLC.

    -         Invoices are delivered by mail or email and are due upon receipt.

    -         After 15 days of non-payment late fees may apply. After 30 days of non-payment, services will be paused until payment is collected in full. 

    -         For those clients utilizing insurance benefits, at times insurance may deny reimbursement. The client is then fully responsible for those services not reimbursed by insurance.

    -         Hourly rates are billed in 15 minute increments, rounded up after 7 minutes into the quarter hour. For example, a session that starts at 9 am and runs until 10:08 will be billed for 1 hour and 15 minutes.

    -         Services to be billed for are: face to face contacts, program development, video chats, telephone conversations, assessment summaries and reports.

    -         Clients may also be responsible for incidental costs, including but not limited to travel and materials necessary for implementation of treatment.

    -         You agree to reimburse us the fees of any collection agency, which may be based on a percentage at a maximum of 50 % of the debt, and all costs, and expenses, including reasonable attorneys’ fees, we incur in such collection efforts.

    Rates: 

    *Clients using insurance.  BTLC will submit claims to your insurance company.  BTLC has contracts with each third-party payor and all guidelines, rates and codes differ depending on your third-party payor and your specific plan (if utilizing insurance).  If you wish to use insurance you MUST submit a Verification of Benefits through BTLC.  You are responsible for notifying BTLC of ANY changes to your insurance plan.  You are also responsible for any fees your third-party payor denies.

    All services provided under 9 hrs. per week are the standard rate of $55 per hour. BCBA consultation services are billed at $115 per hour. 

    Clients discuss with their BCBA how many hours are necessary for optimal progress. Once a number of sessions are determined we will work with each family to choose which sessions would work best, all scheduling is subject to therapist availability.

  • Client Rights

    Parent/Guardians retain the right to refuse any proposed treatment plan.

    Clients have a right to confidentiality. Unless it is not feasible or is contraindicated, the discussion of confidentiality occurs at the outset of the relationship and thereafter as new circumstances may warrant.

    BCBAs provide behavioral diagnostic, therapeutic, teaching, research, supervisory, consultative, or other behavior analytic services only in the context of a defined, professional or scientific relationship or role.
