Our team at Thousand Oaks Smile Design is excited to welcome you back!. It is important for you to know that we are taking great care to ensure the safety of our patients as well as our staff and details of our multi-faceted "COVID-19 Safety Plan" can be found on our website.
Prior to your visit, please complete the following REQUIRED questionnaire and consent forms. It is critical that each patient undergo a robust screening prior to admission into our office. Your patience is greatly appreciated as we take these necessary steps for the safety of everyone. Please be truthful and candid in your answers.
Please note that all visits are now subject to an $18 dollar fee for implementing the CDC recommended personal protective equipment ("PPE Fee") and special precautions being taken to maintain the safety of our staff and patients as outlined in our "COVID-19 Safety plan". This may or may not be covered by insurance and is in addition to any fees normally associated with your visit.
We do still ask that you give us as at least 48 hours notice if you are unable to keep your reservation due to reasons related to COVID-19.
