  • The following documents are required before approval to volunteer at an AFM project:

    • AFM Volunteer Application (this document)
      Proof of travel insurance which includes health/ accidental death & dismemberment
    • Signed/Witnessed Liability Waiver
    • Criminal Background Check (either provide verification of clear background check (such as Verified Volunteer) OR sign forms for AFM to process a background check (for a fee)
    • Read the AFM Child Protection Policy
    • Completed Child Protection Training/Quiz
    • Pastoral Reference Form
    • Sign the Statement of Ethical Foundations
    • Sign the AFM Statement of Faith and Final Authority

  • Proof of Insurance

    Provide Proof of Health/Travel/Death/Dismemberment Insurance
  • Health insurance is required for all AFM Direct Service Volunteers.  For AFM Direct Service Field Project Volunteers, accidental death and dismemberment insurance are also required in case of illness or major catastrophe on the project. Death and dismemberment insurance can be purchased along with the plane ticket. 

    Sometimes a local church or conference will decide to help sponsor an AFM Direct Service Field Project Volunteer and vote their support at a local church board or conference.  In this case, it may be possible for them to secure such insurance provided by the denomination at a reasonable rate through Adventist Risk Management.

    Submit your proof of insurance by providing a copy of your insurance card, contract, or receipt of purchase.  Persons covered and dates of coverage must be specified.
