Bay Street Pediatrics Financial Policy 2024

What to expect upon arrival:
Co-Pays and Balances/Credit Cards on File: Co-pays are your contract with your insurance company. Co-pays are due on the date of service or you will incur a late co-pay fee. For your convenience we accept cash, checks, and all major credit cards.
It is now our practice policy to require all families to keep a credit card on file with us. This allows us to promptly collect the amount your insurance carrier says is your responsibility (including any applicable fees as detailed in this letter). All fees for non-covered services, $200 and less, will be charged to your credit card on file once your explanation of benefits is received by the billing office. Your credit card information will be kept confidential and secure as required by the PCI Securities Standards Council.
Insurance Cards: Due to frequent changes in insurance coverage, please be prepared to provide your insurance card to the front desk at every visit. HMO plans require that you select your primary care provider. If the assigned Doctor’s name on your card is not one of our doctors, your visit will not be covered. Please call the number on the back of your card to formally select our practice. We will file for payment from your insurance company. If insurance coverage is not actually in place at the time of a visit, you will be responsible for paying in full with a cash discount.
Missed Appointments: As a courtesy, we provide text, email, or phone call reminders of your scheduled well visit appointments. If you are unable to keep your appointment, kindly let us know so that we can help you to reschedule. Due to staffing costs, missed appointments (without the courtesy of a telephone call 24 business hours prior to that appointment) will result in a $25 missed sick or medication follow up appointment charge or a $50 missed well visit charge. Emergencies will be considered on a case-by-case basis for waiver of any fees.
Returned Checks: A $20.00 fee will be charged to your account for returned check that are returned by your bank. After two returned checks have been returned on your account, we will request payment by cash or credit card only.
Nonpayment: It is understood that if your account is turned over to a collection agency, you will be responsible for any collection costs that are incurred. Once an account is sent to collections, you will be
discharged from the practice and will have 30 days to find new medical care. Remember that payment arrangements can be made at any point during this process prior to the account being sent to a collection agency. However, once this step has been taken, we cannot reverse the process of collections nor the discharge from the practice.
Comprehensive Administrative Plan “CAP” Fees: a yearly fee $50/patient with a family maximum of $150.00 billed yearly on October 1st. CAP allows us to provide all services not billable through your insurance company under a single yearly fee. A late fee of $10 will be applied for all CAP fees not paid by December 1.
Well Visits: Well visits are an excellent opportunity to review your child’s overall health and development, behavior, school performance, puberty, etc. We recommend that your child have a well visit every year to ensure his/her good health. Please keep in mind that children presenting for a well-physical with a list of medical concerns and/or medication follow up requiring the doctor’s attention may be charged a co-pay or charges being applied to your annual deductible or you may be asked by your doctor to return for an additional visit specifically to tend to ongoing concerns. All children must have an up-to-date physical for their age before any refills, booster vaccines, or form completions can be provided. In accordance to the American Academy of Pediatrics it is our policy to require a well physical with all recommended vaccines for every child once a year. All well visit paperwork needing completion for each of your child’s physicals is listed on our website or patient portal.
Vision and Hearing Services: Bay Street Pediatrics highly recommends hearing and vision screenings for your child each year. If you choose not to have this done at their physical, we recommend your child see a specialist for these tests. Bay Street Pediatrics will bill your insurance company for the services that may be provided. If it is determined that these services are not eligible for coverage, you will be responsible for payment.
Plusoptix- (99177) $30.00Plusoptix screens for astigmatism, myopia, hyperopia, anisometrophia, strabismus, amblyopia, anisocoria. Our screening tool Plusoptix uses an infrared camera that combines auto refraction and video retinoscopy. The camera is used to take a digital photograph; the information acquired is used to automatically assess a child’s vision in as little as five seconds.
OAE Hearing Test (92587) $95.00An Otoacoustic emissions hearing test. A small probe is placed in the ear. If the cochlea is functioning properly, it should echo in response to the sound. If your child fails the test, you might be referred to an audiologist for further testing.
Non-Covered Services: I understand my child may receive services that may or may not be covered by my insurance company. Please note the following services and fees that may not be covered, but are not limited to, as follows:
EKG baseline screening - $25.00
Pre-Travel Consultation - $65.00
You have the right to waive any service you do not wish your child to receive. Please just inform the front desk or nurse at your visit prior to any testing being performed.
