1. The customer hereby agrees to the Village’s rules and regulations. The customer hereby makes this application for water & sewer service at the address indicated above and agrees to pay for such water & sewer services at rates established by the Village, which are reviewed from time to time and at least annually. The current monthly rates are as follows:

2. The Village agrees to provide water service to the customer subject to the provisions of Village Ordinances, rules and regulations for the operation of the water system.
3. The water meter/s on the premises (including electronic meter reading system) shall remain the property of the Village and may be removed by the Village upon termination of the services to the premises.
4.There may be a utility easement on your property. Prior to altering the grading or elevation of your lot, contact the Village Hall or the Building Inspector to inquire as to what changes may or may not be allowed concerning that easement.
5.There is a $150.00 Water Deposit which is required per Village Ordinance. This must be paid at the time of application.
6. Payments are due on or before the 15th of each month. There is a 20% fee for late payments.
