Salon Service Pre-screening:

Symptoms of COVID-19 include but not limited to:
- Fever
- Fatigue
- Dry cough
- Difficulty breathing/Shortness of breath
- Sore throat
- Muscle aches and pains
- Diarrhea
- Loss of smell and/or taste
- Chills
I confirm the following:*

I am not presenting any of the above mentioned COVID-19 symptoms, nor have I experienced the symptoms listed above within the last 14 days.I have not been diagnosed with COVID-19.I have not knowingly been exposed or in close contact to anyone diagnosed with COVID-19. Close contact is defined as: being within 6 feet of a COVID-19 case; caring for, living with, visiting, sharing a small space with a COVID-19 case; having direct contact with infectious particles (ex. direct sneeze) from a COVID-19 case.I have not traveled outside the United States in the past 14 days.I understand in order to receive a salon service, I must wear a face mask or a face shield.I do not have a temperature and will verify.
Due to the 2019-2020 outbreak of the novel Coronavirus (COVID-19), Avant, Incorporated has implemented enhanced sanitation/disinfection protocol. Even following strict sanitation protocol, it is still possible to contract COVID-19 while at Avant, Incorporated. Avant, Incorporated will follow all recommended guidelines to minimize the risk of transmission. Avant, Incorporated reserves the right to turn away any guest that visibly presents symptoms as described above or has not checked a box above.

In addition, the Avant team is screened daily upon arrival and has to fill out the same form as the guest. Any team member unable to check all boxes or exhibiting described symptoms will not be allowed inside the salon or at work until they test negative for COVID-19 or are symptom free for 72 hours without using medicine to reduce fever. If you are scheduled to receive a service(s) from a team member who is unable to work, we will offer another team member to deliver the service(s) or we will reschedule your appointment.

Guest Liability Release:

I understand and accept that there is a risk in agreeing to receive a haircut, hair treatment, chemical service, or facial waxing during the COVID-19 pandemic. Protecting against contracting COVID-19 is made difficult because what is now known is that the virus has a long incubation period during which carriers of this virus may not show symptoms and be highly contagious, thus presenting a risk of infection to all who are near them, or who touch surfaces contaminated by them. One means of protecting against the virus is to maintain social distancing guidelines of 6 feet or more. Maintaining social distancing is not possible when receiving the services provided at Avant, Incorporated. I accept the risk associated with my presence at the salon and receiving the requested services.

By signing below, I agree to each statement above and release Avant, Incorporated from any and all liability for the unintentional exposure or harm due to COVID-19. In addition, if I take any steps to collect damages against Avant salon, its agents, or employees, for receipt of a service during my visit, I am obligated to pay all legal fees, costs and expenses incurred by Avant, Incorporated as a result of my claim.
