Due to the outbreak of the novel Coronavirus (COVID-19), Latora D. Hair Studio, operated by Latora D Gaston, is taking extra precautions with the intake of each client. To protect you, other clients, and our community, we will be following the Center of Disease Control (CDC) and local health department guidelines. Please review, complete and sign form below.

• Only customers receiving services will be allowed in the salon suite. Due to capacity restrictions, we are not able to allow anyone to wait in the seating areas or any other area inside the building. You must wait in your car for your appointment. Please text me at 972-589-4326 when you arrive.
• Mask will be required and must be worn at all times while in the build and in my suite.
• Hands must be washed and/or sanitized upon arrival of your appointment and after using the restrooms. Hand  sanitizer dispensers have been placed at the entry of the building. I will also have hand Sanitizer for you to use upon entering my suite and for the duration of your service. 
• Temperatures will be taken with a contactless thermomter before entering my suite.

WHAT I AM DOING TO KEEP MYSELF AND CLIENTS SAFE: I will be wearing a mask at all times. I will also wear a shield for any services close to my face. My suite, high touch surfaces and implements will be thoroughly sanitized frequently after each client. I will be frequently washing my hands and using hand sanitizer. I will be using as many disposable items as I can. I will be checking my temperature daily. I have updated certification on sanitation protocols via Barbicide and OSHA. I will only be accepting contactless payments. 

 Symptoms of COVID-19 include:

• Fever 100°F or above
• Sore throat 
• Dry cough
• Difficulty breathing/respiratory issues 
• Fatigue 
• Chills
• Muscle pain 
• Lost of smell or taste
• Headache

By completing, signing, and submitting the form below, you understand the above symptoms, comply with the written instructions above, and affirm the following statements below.

1. I affirm that I, or any members of my household, do not currently, nor have experienced the symptoms listed above within the last 14 days.
2. I affirm that I, or any members of my household, have not knowingly been exsposed to, or been diagnosed with COVID-19 with in the last 30 days.
3. I understand that I am at the salon at my own risk and that the 6 feet distancing recommend by the CDC is not possible when seeking hair services. 
4. I understand that Latora D. Gaston (Latora D. Hair Studio) can not be held liable for any claims and liabilities related to COVID-19.
