Waiver and Release of Liability

Buttercup Pole Dance is an 18+ Facility. 

I agree to the following release of liability as a participant in classes, parties, private training or any other activities/or use of equipment at Buttercup Pole Dance, LLC.

All exercise, including the use of poles, aerial silks, lyra, and aerial slings as equipment is inherently dangerous and shall be at the sole risk of the student/participant. The studio, its owner(s), employees and officers are released and held harmless of any claim brought forward by the students, executor, or any other person who may claim on their behalf.

By signing this release of liability, I acknowledge that pole dancing and pole fitness involve risk and the possibility of serious injury, property damage, or death and therefore fully and freely assume these risks and understand the hazards involved.

 

Late Cancel and No-Show Policy

• You may join the wait list for a class that is full, if you get in you will get an email, if you do not get in the credit* will be returned to your account.
• Please be prepared to attend class if you are on the wait list, sometimes notice is given without much time before class.
• A cancellation within 24 hours of class time is considered "late" and that credit will be forfeited. 
• "No shows" will forfeit credit and pay a $10.00 fee.
• Buttercup has a no refund policy.
• ALL packages have a 90 day expiration date from time of purchase.
  Contract memberships must be fulfilled and will not be stopped unless the client sustains an injury or moves 35+ miles from the studio.

*Money paid for credits is not returned - however the credit for the class is returned to your account on Mindbody and can be used again until the expiration date.

 

Video Recording Policy

·         No recording of class is allowed until the last 10 minutes of the session.

·         Recording of instruction is strictly forbidden.

·         If you’re not a member of a class, no recording is permitted at all.

·         If you plan on posting your video please make sure you have permission from those included, give credit to the studio, and give credit to the choreographer.

I acknowledge that I have read and fully understand this agreement and intend my signature to be an unconditional and complete release of all liability.
