Eye exam and treatment fees are payable at the time of service. If filing insurance, we will gladly do so for your visit with your insurance plan. There are two types of insurance plans. Below is to help you understand how your visit will be filed today.
Vision Care Plans:
- Covers eye wellness exam only.
- Some plans may have a benefit for glasses or contact lenses.
Medical Insurance Plans:
- Filed if a medical condition is diagnosed and treatment plan is prescribed by the doctor or if you have a systemic health condition that could have ocular complications. Examples of these conditions: Diabetes, Glaucoma, Cataracts, Macular Degeneration, Dry Eye Disease, allergies, eye infections, flashes and/or floaters, sudden loss of vision, etc).
- All services performed under the medical insurance plan are billed according to the complexity of care given at the time of service.
At the time of service, you are responsible for payment of deductibles, co-pays, co-insurance, and non-covered services that are dictated by your vision care plan and/or medical insurance plans. Our staff may assist you in estimating your insurance benefits, however they are never guaranteed. If you are covered by any form of managed care insurance that requires prior authorization for our services, you are responsible for obtaining prior authorization. We will apply a 1% statement charge each month to all accounts that are over 60 days past due. Any account that is over 90 days past due will be sent to a collection agency.
