    Project and Event Related Forms
  • Instructions:

    1. Fill out all required fields.
    2. Make sure the person filling out this form is the person-in-charge of the project.
    3. Supporting documents such as concept notes, GD mechanics, etc. may be attached if they will help in the expedient processing of the project proposal.
    4. Submit the proposal to the Office of Student Activities (OSA) at least one (1) month before the intended date of implementation. This is to ensure that any logistical and financial concerns can be secured at a reasonable time.
    5. Incomplete proposals will not be accepted.
    6. Processing of project proposals by OSA takes at least one (1) clear working day.
    7. If approved, proceed with the next steps of the activity (Accounting Office for finance). If pending or disapproved, act on the remarks, comments, suggestions, and additional requirements needed as stated on the retrieved document. Consult with your OSA Professional if necessary.
    8. In cases when proposals are returned for clarifications and/or require consultation with the organization regarding the project details, the proposal will be approved only after the concerns are addressed. If there are necessary revisions on the form, attach the first draft with the new submission.

     

    REMINDER:

    Be informed that all PPFs should be submitted 7 school days prior to the target activity date for big venue-level events.

    Activities submitted less than the days mentioned above will be automatically disapproved and advised to resubmit with a new schedule.

    Your proposed activity will be held in person. Please note that the following steps are required before completing this form:

    1. Read the Guidelines for Onsite Student Activities.

    2. Appointment of an Organization Safety and Compliance Liaison Officer (OSCLO).

    After carefully reading the guidelines, please appoint an OSCLO and provide their name / contact info on the spaces provided on the last page of this form.

    Additional step after submitting this form:

    3. Individual Consultation (IC) with your OSA Formator. 

    An IC is required for in person activities prior to approval. After submitting your PPF using this form, please contact your OSA formator via email to schedule an IC.
