This form is a formal agreement between you (the client) and Crowned Luxury, LLC (the business), for both of your protection. Please only fill this form out if you have placed an order online or have sent hair/extensions to me personally. Please fill this form out in its entirety. It will need to be filled out prior to wig construction beginning. Do not leave any info blank on this form. If something does not apply to you, simply put "N/A." Once the form is received, you will receive an email to pay the deposit which is 50% of the total cost. This can be paid by Venmo or PayPal. This form will always be kept on file.
