    Payment:
Payment is due in full at the time of session. Dr. Dorsky accepts cash, checks, and most major credit cards. He does not accept American Express. Payment not received at the time of the appointment will be subject to a $50 late fee.

    Communications:
All communication with Dr. Dorsky is by phone. Dr. Dorsky does not text or email with patients.

    Cancellations:
    See attendance policy consent form.

    Weather:
If the office is closed due to inclement weather or other emergencies, every effort will be made to notify you in advance.

    Prescription Renewals:
See medication refill policy consent form.

    Abuse and Neglect:
State law mandates the physician to file a report to DYFS in any cases where the physician has reasonable suspicion of physical or sexual abuse or of neglect of the child. In these cases, normal confidentiality will be broken.
