You are seeking treatment at the office of Dr. Seth Dorsky. Your treatment plan calls for regularly scheduled visits. Attendance at scheduled appointments is very important for successful treatment.
These are Dr. Dorsky’s policies about keeping appointments:

Please be
on time
for your/your child’s appointments.
Please allow enough time to travel to our office.
If you are late for your appointment, Dr. Dorsky will have less time to spend with you/your child. If there is not enough time to complete the treatment planned for that session, the appointment may have to be rescheduled.

If you need to cancel, change, or reschedule your/your child’s evaluation, you must call 973-635-4244 at least 48 business hours in advance.
If the requested notice is not provided, you will be responsible for paying half the cost of the intake appointment.
This allows us time to give your intake appointment to another patient who needs one.

If you do not attend your appointment, it is called a
missed appointment. First missed appointments are forgiven. For the second missed appointment, you will be responsible for paying the full price of the appointment. Payment for the missed appointment will be due at your next appointment along with the payment for that session. If a third missed appointment occurs, this office will be unable to continue providing services.

If you/your child misses 3 appointments, you will be notified in writing that this practice is unable to continue to provide services. You will be provided with information about other places to receive treatment that may be more convenient for youyour family.

We understand that emergencies occasionally happen at the last minute. However, if there is a
pattern
of last minute cancellations within 48 hours:
• Dr. Dorsky will review your/your child's treatment and will decide if it is appropriate to continue providing services to you/your child.
• If Dr. Dorsky is unable to continue providing services, you will be notified in writing. Dr. Dorsky will provide you with information about other places to receive treatment that may be more convenient for you/your family.
