I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Stacey James Photography LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Stacey James Photography LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other clients and their families.
I voluntarily seek services provided by Stacey James Photography LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while in proximity.

Due to Covid -19 Stacey James Photography is taking more protective measures to keep herself and her clients safe. Safety is my highest priority during this time. Please read ALL and sign at the bottom. 

 

1) I agree that if I am showing ANY signs of illness I will contact Stacey James Photography ASAP and reschedule our photo session. 

 

2) I agree for studio sessions ONLY immediate household/ family will be present. 

 

3) I understand for studio sessions that shoes will not be allowed in the studio.

 

4) I understand for studio sessions pets are not allowed. 

 

5) I understand for studio sessions, Stacey James Photography washes and disinfects all props, fabrics, outfits etc to the best of her ability. 

 

6) I understand that if Stacey James Photography has any symptoms of being sick she will reschedule your session. 

 

7) I understand that all parties will do their best to cover their noses/mouths if they sneeze or cough.

 

8) I understand that Stacey James Photography will not be held liable if anyone in my party develops covid-19 or symptoms of illness any time after our session has occured. Stacey James Photography, LLC cannot be sued, or held responsable for any illness developed post session. 

 

9) I understand that if I do develop symptoms or test positive for Covid-19 I will contact Stacey James Photography as soon as possible to alert her. 

 

 

I hereby release and agree to hold Stacey James Photography LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of services, or that may otherwise arise in any way in connection with any services received from Stacey James Photography LLC. I understand that this release discharges Stacey James Photography LLC from any liability or claim that I, my heirs, or any personal representatives may have against the photographer  with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Stacey James Photography LLC. 

 

This release form is a blanket release and reigns govern over all those that attend any function or event that Stacey James Photography LLC has been hired to photograph.
