I understand and acknowledge that this is a general form used by my stylist to help me choose the correct booking option so that I can achieve my hair goal. I understand and agree that if I would like a more detailed and complete analysis of my current hair, and if I would like a more thorough consultation that I would be required to book an in-person consultation.

I understand and acknowledge that by submitting this form, I am NOT booking my appointment at Cat Keyes Salon and that I am submitting a consultation form and a request to book in the near future. I understand that Cat Keyes Salon typically has appointments booked out 6-8 weeks in advance and I may not be able to have my appointment before that time frame.

I understand that typically the initial investment for a first appointment or a color transformation can vary from $200+ and that maintenance appointment prices vary depending on my current hair.

I understand that maintenance appointment prices are for appointments booked no later than 4 months after an initial Color Transformation appointment/a New Client Appointment/ Previous on schedule hair appointment session. I understand that going 5 months or longer between my appointments requires me to book an new initial service because I have missed the maintenance appointment window.
