I, the undersigned President/Chief Officer of the organization listed below (The Organization), understand and agree that The Organization shall adhere to all rules, regulations, policies, agreements and decisions of the Northern Illinois Soccer League (NISL). Ignorance of the rules, regulations, policies, agreements and decisions of the NISL shall not be an excuse for failure to comply with same.
I am aware there are risks to the participants in The Organization activities of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof. COVID-19 which has been declared a worldwide pandemic by the World Health is extremely contagious and is believed to spread mainly from person-to-person contact.
I understand and fully agree that NISL does not provide any insurance coverage for Covid-19 related claims. I certify that The Organization will collect from each participant prior to participating in The Organization’s activities a signed “NISL Communicable Disease Release of Liability and Assumption of Risk Agreement”. The executed Agreement shall be deemed an important document of The Organization and will be retained for a minimum of seven (7) years and made available to NISL upon request.
I certify that The Organization will communicate to the parents/guardians, prior to initial participation insurance information, protocol and responsibilities including its own preventive measures that comply with all state and federal laws including but not limited to the State of Illinois and local government public health orders, Centers for Disease Control guidelines and “NISL COVID-19 Return To Play Guidelines”. I certify that prior to an individual participating in any in-person training or tryouts The Organization will obtain a completed “NISL Communicable Disease Release of Liability and Assumption of Risk Agreement”.
I have read the “NISL COVID-19 Return To Play Guidelines” and fully understand that the information in it is NOT intended or implied to be a substitute for professional medical advice, diagnosis, or treatment or legal advise. The protocol addresses only early phases of return-to-activity and additional guidelines may, or may not, follow as the COVID-19 pandemic continues to evolve. The knowledge and circumstances around COVID-19 are changing constantly and, as such, NISL makes no representation and assumes no liability and/or responsibility for the completeness of the information. Further, The Organization is expected to strictly comply with the Center for Disease Control (CDC), state and local guidelines for the resumption of play and seek advice from medical professionals and/or public health officials in its area.
